Q.1: What are the membership eligibility requirements?

All Muslim Canadian Citizens, Landed Immigrants holding Permanent Resident Card and legally staying in Canada and their family members are eligible to register as a member of FSSO.

Q.2: What is needed to become a member of FSSO?

You have to simply fill up a membership application form and pay one time membership fee of $100 plus $25 admin fee as Principal Applicant, $25 for spouse and $25 for each dependent child under the age of 19 years as their membership fee. Child over 19 years of age has to fill up separate membership form as an Adult Principal Member and has to pay one time membership fee of $100 plus $25 admin fee

Q.3: Is there any payment I have to make once I become a registered member of FSSO?

Yes, if any registered member of FSSO passed away only then you as a principal member will be required to share proportionately the funeral cost incurred for the deceased member to reimburse the stabilized balance of Membership Account. Your reimbursement share, which will not exceed $15 at one time, will be withdrawn from your bank account as per Pre-authorized Payment Plan Payment Agreement signed by you.

As unanimously agreed by Management and FSSO members, an amount of $15 will be withdrawn
in a year on 1 st January being contribution of member for FSSO office overhead expanses

Q.4: Is there any additional facilities or requirement for Senior Citizens of age 65 and over?

Effective 1st March, 2020 Membership Application from Senior Citizens of age 65 and over will be accepted on the condition that FSSO will provide full funeral support services but will bear the funeral cost up to maximum $3,000 of the total amount and the rest will have to be paid by the family of deceased member which is reimbursable by the Canadian Government.

Q.5: If I move to any other city of Ontario Province will I still be entitled for the benefit of FSSO services?

Yes, you will continue receiving benefit of FSSO services as long as you are within Ontario Province area where funeral services arrangements are available in the nearest masjid and Muslim grave yard facility is there.

Q.6: Will I be eligible for full FSSO services immediately after my registration a member?

As per established policy of FSSO the membership activation period is three months from the date the application form is received. After three months the membership will automatically be activated for full FSSO services.

Q.7: During my membership activation period will you deduct my share of reimbursement in the event of demise of any FSSO member?

Yes, as a registered member you will be required to pay your share of reimbursement.

Q.9: Can principal applicant add the name of parents in his/her application form?

No, parents can not be added in principal applicant form. If they meet membership eligibility requirement they have to submit separate application form.

Q.10: In the event of FSSO member’s death, what procedure should be followed?

You have to simply give us a call at our number 289-632-6362 and follow our instructions. If you want more detail, please go to our website “www.fsso.ca” and read the procedure outlined on page 2 of “Journey of FSSO”

Q.13: I am a registered Principal Member of FSSO. One of my child who was also registered as a child member of FSSO at the time of my registration has reached the age of 19–what next I have to do?

You have to fill up a separate membership Application Form for him/her which can be downloaded from this website and send to us with $100 Membership Fee minus $25 (already paid when he/she was registered as dependent child) plus $25 Admin Fee to register as Principal Applicant. Please make sure that you or your child (if has a separate bank account) fill up and sign the “Pre-Authorization Payment Plan Agreement “portion on the back of the application form and attach a VOID cheque.

Q.15; If the applicant and his/her family living in remote area out of GTA how FSSO will make funeral arrangement and payment there if any member passes away.

The family must look for a Masjid with funeral arrangement and Cemetery where Muslim Section is available and notify us. For the payment of funeral cost up to $6,000 minus applicable overhead expenses, it is the responsibility of the family, the Masjid and/or the cemetery of that area to ensure that the invoice/s are issued in the name of Funeral Support Services Organization and submitted within 10 days from the date of burial, otherwise the funeral cost will not be reimbursed after 10 days.

Q. 16; If the new applicant Principal Member does not include the name/s of his/her spouse and children under 19 years at the first stage when submitting the Membership Form for the reasons known to him/her can he/she add their name/s later on.

Yes, they can include them by paying membership fee of $25 + $ 25 admin fee each and by filling up the “B” Form available on FSSO website but they will also be subject to 3 months activation period from the date their name/s included in the dependent list of principal applicant.
